This information is provided as a general synopsis of the different levels of care provided at Colonial Terrace Nursing Center. For more information about these levels of care, please contact us.

Skilled Care • This is the highest level of care available in a nursing center. Our Skilled Care residents require services by a licensed nurse or therapist on a daily basis. Assistance is given in all aspects of care including bathing, dressing, grooming, meals, medication administration, licensed therapy programs and much more. Many residents utilize this level of care when first entering Colonial Terrace from a acute care hospital. Many are able to eventually go to a lower level of care or in some cases, return home after receiving their course of treatment. Care is planned and delivered by a trained interdisciplinary team.

Nursing Facility Care • Similar to Skilled Care however the residents on this level of care require more routine nursing and care services. Licensed therapy can be given but the resident does not require it on a daily basis. Rehab services, assistance with bathing, dressing, grooming, meals, activities and medications are still given. This level of care is often referred to as “long-term-care,” but can also be utilized for short respite or Hospice services.

Personal Care • Residents on this level of care are the most independent. They are able to walk or mobilize themselves in their wheelchairs but continue to receive minimal assistance with their normal activities of daily living including medication administration. Resident on Personal Care enjoy more freedom of independence while they and their family members have peace of mind knowing that a caregiver is always nearby.
